[B] Scope of Service:

Vendor needs to provide case management, third party billing, technology services including providing assistance with cooking and developing cooking skills; providing assistance with household cleaning and developing these skills; providing assistance with maintaining a daily schedule/routine and developing these skills; providing assistance with money management skills and developing these skills; providing assistance with medication administration and developing these skills; providing assistance with all other activities of daily living skills and developing these skills. The majority of services will be provided in the client’s residence although some services may be provided in the community.
- Supporting living services activities include, but are not limited to:
• Evaluating clients to determine need
• Providing in-home services such as medication administration/monitoring, activities of daily living skill building and money management
• Meal planning and nutrition guidance
- Case Management – A case manager coordinates the services for all clients on Supervised Release (SR) and provides pre-planning, assessments and communication for clients pending release (DHS ordered to prepare a SR Plan) at Sand Ridge Secure Treatment Center (SRSTC). For some clients this will be minimal, for others this will be extensive due to possible physical, cognitive, and psychiatric needs. A case manager serves as a liaison between community providers and assures that the client’s needs are being met. Case managers are on-call and respond to routine issues. This includes during business hours and after hours. Case managers must balance the needs of the client with the understanding of community safety, state statute, program policy/procedures and the best interests of the SR Program.
Case management activities include, but are not limited to:
• Pre-release planning, assessment and communication
• Case planning
• Community placement
• Oversight of client activities and requests
• Coordinating, scheduling and leading client staffings and case consultations
• Home visits, telephone contacts and collateral contacts
• Client financials including purchase requests and financial violations
• Assisting with the coordination of short-term and long-term care services
• Housing set-up, shutdown, walk-through and maintenance coordination
• Resource assistance with basic needs
- Third Party Billing Services– Clients in the community receive services and incur expenses that are the responsibility of the client to pay using income, benefits, and assets. DHS is statutorily responsible for the care, custody, and treatment of clients on Supervised Release. If a client is unable to pay for their services or expenses and it is considered statutorily mandated, then DHS will pay. In fiscal year 2019, DHS processed invoices totaling over $5,000,000 million for Supervised Release community services and expenses. DHS will contract for the processing and payment of all invoices.
Third-party billing activities include, but are not limited to:
• Processing invoices for client expenses such as rent, utilities, and required services
• Paying invoices in a timely manner
• Processing client contributions to the cost of their care and properly accounting for these payments
• Send out expense checks for client’s daily living needs as needed
• Ensure necessary utility contracts are in place when lease agreements start and discontinued when lease agreements are ended
- Technology Services – Clients on SR are unable to have personal devices that have internet connectivity due to their rules of supervision. As a result, DHS will contract for a technology platform to deliver behavioral and/or health care services to a program residence. This platform must include the costs associated with the purchase of technology devices and protecting, securing, and maintaining such devices (i.e. protective coverings, security software, software updates, device upgrades, etc.).
This technology platform must include, but is not limited to:
• The ability to deliver telecommunication and video conferencing technologies in a safe and secure manner to client’s in their residence
• Compliance with DHS BAA (Business Associate Agreement), court approved rules, HIPAA and Supervised Release Program policy, procedures and guidelines
• The ability to lock down the device so it only allows the client to take part in prescribed teleconferences, does not allow any other video or audio calls and does not allow any internet access other than the access required to participate in the prescribed teleconferences
