Vendor needs to provide life, disability and leaves management services to the government authority located in Lane, OR.
- Obtain medical evidence needed to evaluate claims in accordance with federal or state law.
- Accept new claim requests through a toll-free customer service number or web-based service.
- Advise the county as to the continued status of employees and ability to return to work in full capacity or modified duty.
- Obtain necessary release of medical information paperwork from employees.
- Assist employees regarding plan eligibility and application for other relevant benefits provided by the county or state or federal law.
- Administer claims review or appeals in accordance with collective bargaining agreements, county policy, plan document, federal law and state law. Provide follow-up information as needed to obtain additional information from employee and healthcare provider.
- Maintain complete and accurate tracking for purposes of establishing employee eligibility and utilization.
- Maintain complete and accurate historical tracking information for employee leave.
- Coordinate return to work with employee and the county prior to the scheduled end date of a leave of absence.
- Assign a leave of absence specialist to each claim.
