[*] Scope of Service:

(1) Vendor needs to provide visa, MasterCard card incentive benefit services for child wellness incentive pilot program to the government authority located in Hawaii.
- Prepaid branded child wellness incentive program gift cards must be accepted by a wide variety of locations and businesses throughout the workforce service areas, throughout the Hawaiian Islands
- Must designate child wellness incentive program branding on each visa or Mastercard card
- Each card must be issued in the fixed amount of Fifty Dollars with declining value with each usage.
- Agency expects to order approximately 100-500 cards per month.
- The cards must not allow a cash-back option
- The cards must not have an expiration date.
- The cards must be sequentially numbered to facilitate the distribution, tracking, and
reconciliation of the cards.
- The cards must have the alcohol, tobacco, and firearms purchasing restrictions
(2) All questions must be submitted no later than November 6, 2023.
(3) A notice of intention submitted no later than November 3, 2023.
