[*] Scope of Service:

(1) Vendor needs to provide health flex spending, dependent care assistance, cobra billing, retiree billing and direct bill services
- Employees contribute up to the IRS maximum of annually to the health care account
- Employees contribute up to the IRs maximum annually to dependent care accounts
- Employees have the option of either a manual claims submission process or a point-of-purchase reimbursement through a debit card
- Direct deposit for manual claims reimbursements
- Employees have up to 90 days after the end of the plan year to file claims
- access to all participants for the Health Care Reimbursement and/or Dependent Care accounts to review their claim status, balances, et
- Prepare and furnish eligible employees with communications material for enrollment purposes that define all covered expenses, limitations and exclusions, procedures for filing claims, and reimbursement procedures. Maintain central claims and membership files and records of claims reimbursements for each covered member
- Direct deposit of reimbursements in participant bank accounts
- Provide initial FSA Debit card free of charge
- Provide quarterly and year-end statement reporting of account balance to participants
- Provide monthly management reports to the City
- Develop and distribute a Plan Document/SPD for the Flexible Spending Account Program
- Provide manual and debit card point-of-service options for claims reimbursement.
- Provide internet access to participants for Health Care Reimbursement and/or Dependent Care Accounts to review claims status, balances, etc.
(2) A pre-proposal meeting will be held on November 7, 2023.
