(1) Vendor needs to provide representative payee debit card to the government authority located in Albany, NY.   
- The pre-paid debit card process including options for the agency to transmit the money to bank and vendor so it is loaded onto the individual’s debit card each week, and the vendor’s safety protocol for protection of funds transferred to its accounts.
- Identification requirements, if any, to order a preloaded debit card for individual representative payees.
- Processes and timelines to replace a lost, damaged or stolen pre-paid debit card.
- Any upfront costs to establish debit card system.
- Fee schedule for initial debit card, debit card replacement, card closure, transaction fees, etc.
- Overdraft protocols and whether any fees apply.
- For debit cards that are no longer needed, the process of returning any remaining unspent money back to agency of social services so it can be returned to social security administration.
- Whether DSS is able provide restrictions and blocking on pre-paid debit card for over the phone purchases and specific vendor categories include restaurants, grocery stores, convenience stores, bars and liquor stores, casinos, etc.
- The type of online recordkeeping system, which allows the agency to monitor individual card utilization active or inactive, transactions that have occurred, balances on pre-paid debit cards, and manage any need to immediately suspend or close a debit card, request replacement cards, etc.
- The types of monthly, quarterly and annual statements DSS can obtain both by individual client and aggregate representative payee accounts which reflect transactions, deposits, as well as any potential fees incurred. reports should be able to be utilized for social security administration mandated reporting.
- Outline the process DSS would use to access and review individual debit card cases.
- Atm locations in the local area where individuals can use prepaid debit cards to obtain cash, including any fees to make withdrawals.
- Customer service options for pre-paid debit cardholders to check balances, change pin, etc. in person, telephone, online, etc.
- Any additional details regarding the pre-paid debit card implementation and ongoing functioning for representative payee individuals as well as any other costs for reports, or customer service fees that were not included above and on what schedule fees are to be paid.
